Your Enrollment Checklist

Let's Get Started

Step 1 — Confirm Availability

Before completing your full enrollment packet, contact QRT to confirm that space is available in your child's classroom and for the schedule you need.

Step 2 — Complete Required Enrollment Forms

Download and complete the required Missouri childcare forms and QRT enrollment documents listed below.

Step 3 — Submit Required Records

Families may also be required to provide current immunization records, health information, emergency contacts, authorized pickup information, and other documentation applicable to their child's program.

Step 4 — Complete QRT App Setup

Once enrollment is accepted, families will receive instructions for connecting to the childcare management app used by QRT for attendance, communication, billing, daily updates, and other program functions.

Step 5 — Enrollment Review

QRT will review your submitted documents and contact you if anything is missing or additional information is needed.

Step 6 — Welcome to the Royal Family!

Once all required enrollment documents, agreements, fees, and program requirements are complete, we'll prepare for your Treasure's first day.

This is the official Missouri Child Care Enrollment Form and collects information including parent/guardian information, emergency contacts, authorized pickup persons, and other required enrollment information.

🩺 Child Medical Examination Report

This medical examination form is used for infants, toddlers, and preschool-age children and should be completed as required for your child's enrollment.

🏫 School-Age Child Health Report

School-Age Children

Families enrolling school-age children may be required to complete the applicable Missouri school-age health documentation.

💉 Immunization Records

Families must provide the appropriate current immunization documentation or applicable exemption documentation required for childcare attendance in Missouri.

Some documents may not apply to every child. QRT will help families determine which forms are required based on age, schedule, services, and individual needs.

Don't Forget Your Supporting Documents

In addition to completed forms, please have the following records available when applicable:

• Current immunization record or appropriate exemption documentation
• Completed health/medical examination documentation
• Medication or allergy documentation
• Individualized care information, if applicable
• School name and schedule for school-age children
• Transportation information, if transportation is requested
• Custody or court documentation affecting pickup or access, when applicable
• Any additional documentation requested by QRT based on your child's individual needs

QRT may also use Wonderschool for applicable enrollment, program, family, or childcare management services.

Before submitting, please make sure:

• All required sections are completed
• Parent/guardian signatures are included where required
• Health forms have been completed by the appropriate medical provider when required

• Your child's full name appears on all applicable documents
